Instructions:
In order to save time during the assessment process in the first session, please fill out as completely as possible with as much detail as possible the Client Intake Form and also read and sign the Informed Consent form and the HIPAA Consent form. Either scan and email all forms to your therapist at [email protected] or bring them with you to the first session. If this step is not completed prior to your first session, it will be necessary to use the first session to complete them. Thank you.
